    Fall Parent Satisfaction Survey

    We would like to know how the Marshall Food4Kids weekend snack program is working for you. Please take the time to provide your feedback. This survey takes 1-2 minutes to complete. All responses are anonymous and your honest feedback is much appreciated.

    ​About the Marshall Food4Kids program
    Marshall Food4Kids is open to any student in Head Start or Marshall Public School (preschool through 8th grade) who needs extra food assistance for the weekend. With a parent or guardian’s consent, the program sends home a bag of healthy snacks for the weekend. Bags are distributed on the last school day before the weekend or break. This is not a government program; all food is paid for by donations from individuals, churches, businesses and foundations. If you believe your child would benefit from this program, we encourage you to sign them up by filling out this online form or printing this PDF, filling it out, and returning it to your child's teacher or the school counselor.
